Please submit an authorization/referral via your SHC health portal PRIOR to August 1st. To submit an authorization follow the steps below:

  1. Log into your SHC patient portal
  2. Select “Messages” on the left sidebar
  3. Select “New Message”
  4. Select “SHC – Insurance Department: Referral Request and Other General Inquires”
  5. Select “SHC – Referral Request”. Complete the populated fields, then click “Submit.”

 If you have any trouble submitting the authorization/referral, please contact the Student Health Center Insurance Office directly at (949) 824-2388.
